The following instructions outline how to order a Memorial Reef and ship the remains.
Below are 3 different forms needed to process an Eternal Reefs order.
- Disposition Release Form - This form gives Eternal Reefs the authority to mix the cremated remains into the concrete mixture. Once this is done, there is no way to separate the remains from the memorial reef concrete mixture. This is very similar to a cremated remains scattering release, once it is done, it is done.
- Eternal Reefs Order Form - This is the order form that you fill out with your desired memorial reef options.
- Eternal Reefs Policy Statement - This form explains all Eternal Reefs policies including permits and locations, timeframes, multiple sets of remains, reef ball movement and pets.
In your information packet, you will also receive an Eternal Reefs Declaration Form. This form is for families and individuals planning for the future, so please fill out this form and place with your personal papers. Each individual should have their own form and regardless of what memorial you choose, please tell your family and friends so they will know your wishes
